Thank you for your interest in enrolling in Lathlain Primary School.
Please complete an Application for Enrolment Form if wishing to apply for the 2023 or 2024 school year, including the Kindy Program.
In Western Australia children are able to start their education in Kindergarten, however compulsory school commences in Pre-Primary. Please refer to the school age calculator (LINK) to determine your child’s commencement year by their date of birth.
To apply for enrolment please complete an application and submit the form to the school office or via email to Lathlain.ps.enrolments@education.wa.edu.au. Kindergarten enrolments close on the first Friday of Term 3 each year.
Please ensure the following documents are attached to your application:
- A copy of your child’s birth certificate.
- Australian Immunisation Register (AIR) Immunisation History Statement, which is not more than two months’ old.
- Proof of address, in the form of a utility bill, council rates or lease agreement.
- Any other documents listed in the checklist that apply to your situation.
Immunisation Requirements
It is a requirement for you to provide your child’s current Australian Immunisation Register (AIR) Immunisation History Statement as part of the enrolment process.
The statement must be dated within 2 months of the date of application for enrolment. For more information and instructions on how to access your child’s AIR please refer the Department of Health website, click here.
Kindergarten Immunisation Requirements
On 22 July 2019, changes to the Public Health Act 2016 (WA) and the School Education Act 1999 (WA), came into effect that will better protect young and vulnerable WA children and the wider community from vaccine-preventable diseases.
Under these new changes, a child’s immunisation status must be ‘up-to-date’ according to their AIR Immunisation History Statement and must be dated within 2 months of the date of application for enrolment to be permitted to enrol into Kindergarten.
Pre-primary – Year 6 Immunisation Requirements
The AIR Immunisation History Statement must be dated within 2 months of the date of application for enrolment.
